1.      Application for Student Admission (will be provided by this office);

3 ]6 Q4 L8 Q$ r0 f! I, v3.      Birth certificate of child(ren) / Family Register (translated

4.      Proof of residential address / tenancy (eg lease agreement/tax
bill/utility bill - driver’s license not acceptable);
7 ?8 _8 @, M& ~2 L1 h2 q7 M5.      Documentation to verify child(ren)’s first ever date of entry
8 E% t: ]; X/ u: T* H' c& ]into Canada (eg. passport stamp / immigration documents / airline
